I've seen a LOT of poorly spelled names, ie Maddicyn (Madison) or Olyvia (Olivia), but only one that was unrecognizably bad.

As others have said I have seen a lot of tacky spellings but there are a few from the preschool i volunteer at that still shock me

The bad spelling siblings: Binnjamen (Benjamin) {5}, Raychiel (Rachel) {3}, and Theodoore (Theodore) {a few months old} their mother Joy goes so far as to say that unique spellings are her theme.

Xachtairy (Zachary)
Ayvah (Ava)

Then in one class there were 5 masons. Two had the usual spelling. One was Masen, one was Masan, and the last one takes the cake with Maeseynn. (They were all boys)

I honestly haven't even met someone with a terribly spelled name. Maybe small changes like Caden/Kaden/Cayden or something like Mason to Masyn, but nothing extreme.
