I have recently fallen in love with Caterina with the nn Cat.
Questions:
1. Do you think this spelling will cause problems with pronunciation? It is the legit Italian spelling but I think most are used to Katarina.
2. Middle name ideas? Can not end in an "ee" sound. I love Caterina Ivy but anything ending in 'y, i, ie, ee' etc. clashes with the ln.
3. What do you think of the name in general?

1. I dont think changing the spelling to Caterina will present many pronounciation problems. I think this is also a valid spelling.
2. Caterina Iris, Caterina Vivian/Vivienne, Caterina Violet, Caterina Yvonne, Caterina Isla, Caterina Paris.
3. Personally, I am not a fan of names with the hard C/K sound. But Caterina nn Cat is a nice underused name.

I love the name Caterina.
I don't think you should change the spelling, most people won't have any problem with it. After all, there is an equal amount of Catherine's and Katherine's, and people only get confused so much.
MN ideas:
Caterina Mae*
Caterina Isabel
Caterina Violet
Caterina Rose
Caterina Charlotte
Caterina Jane
Caterina Alice
Caterina Vivian
Caterina Piper
Caterina Sophie
Caterina Eve [which is similar to Ivy, maybe it could work as a substitute!]

Thanks for your thoughts! I am glad that the spelling isn't throwing anyone off. I much prefer C's to K's!
I love Caterina Isabel, Caterina Eve and Caterina Violet. I think Rose might be too much R for me. I quite like Caterina Isla too or even Isla Caterina! Will definitley keep these on my list!

I like Caterina. I'm not very fond of K names, very much. I mean, some are nice and are meant to be, but a lot seem to be replacing C's with K's and it doesn't look as good . . . Katerina isn't as nice as Caterina
Caterina Jane
Caterina Maya
Caterina Louise

1. Do you think this spelling will cause problems with pronunciation? It is the legit Italian spelling but I think most are used to Katarina.
Nope, C and K are being interchanged for a lot of names and I'm sure most people would pronounce it perfectly.
2. Middle name ideas? Can not end in an "ee" sound. I love Caterina Ivy but anything ending in 'y, i, ie, ee' etc. clashes with the ln.
Caterina Seren
Caterina Pearl
Caterina Diane
Caterina Isolde
Caterina Jade
Caterina Hazel
Caterina Sarai
Caterina Adele
Caterina Lilith
Caterina Roisin
Caterina Harper
Caterina Isis - personally, I prefer Isis to Ivy and Iris. That's probably just me.
Caterina Esther
Caterina Juliet
Caterina Lenore
3. What do you think of the name in general?
I like it. It's on my long list, along with the similar Catalina.
