Definitely Carol-Ann. However, you should probably drop the hyphen and/or attach an "E" at the end. Carol-Ann vs Carol Anne. See the difference? That hyphen will just be troubling for her for the rest of her life when it comes to legal documents, computer files, etc. Just some food for thought really. :)
