I'd go with Ohara - simpler for filling out forms and honors the Japanese origin. O'Hara only makes me think of Gone with the Wind.

If its baby's first name, I would go with Ohara. It looks less like a last name imo.

Wait- is the family name originally O'Hara, or Ohara? For me that makes a difference, because if you're using a family name, it's better to remain true to the original.

Put it in the middle spot. You need to plan to call him Harry or some other nickname. I would not want too use o'hara on a day to day basis. Some surnames don't lend themselves to be used as first names
If you are not Japanese I would not substitute O'hara for Ohara.
If you are not Japanese I would not substitute O'hara for Ohara.
